For the fastest service in obtaining loss history and loss run reports on an account or policy, please make your request to us via email at:

Please be certain to include the following information in your email request so that we may quickly and accurately respond to your request:

 

  1. Your name and your agency's name

  2. Your email address and telephone number

  3. Name of the account

  4. Provide carrier name(s) and all policy numbers

  5. Specify line(s) of coverage / policy type(s)  (i.e. General Liability, Umbrella, Automobile, etc.)

  6. Specify the coverage term(s) you are requesting (i.e. 5/1/12 - 5/1/13 and 5/1/13 - 5/1/14)
